What happened
On November 15, 2015, a Piper PA-25-235, registration CX-BTH-R, was performing agricultural spraying operations near Route 17, Km 344, in the vicinity of Enrique Martínez, Department of Treinta y Tres. The aircraft, operated by Agro Vuelos Ltda., was conducting liquid application tasks over local farmland.
During the final stages of the mission, while performing a touch-up maneuver toward the northwest, the aircraft struck two electrical distribution cables. One cable grazed the engine cowling, while the left wing struck the second cable, causing it to break. Following the impact, the aircraft descended to the ground, coming to a stop several meters from the point of impact. The pilot evacuated the aircraft on foot and was uninjured. There was no post-crash fire, though two aluminum distribution cables were damaged.
The investigation
The CIAIA investigation examined the flight history, the pilot's credentials, and the operational environment. The pilot was fully qualified and had been performing similar agricultural work in the area for several years. The aircraft was found to be airworthy, with all documentation in order. Meteorological conditions at the time were favorable, with clear visibility (CAVOK).
Investigators also reviewed the use of the onboard AG-NAV P141-Silver GPS. It was noted that there had been several data interruptions throughout the day, likely due to software issues or interference from nearby high-voltage lines. However, the pilot had not reported any operational difficulties prior to the accident.
Findings
- The pilot was experienced and had been adequately rested and nourished.
- The aircraft was in a condition for safe flight.
- The area and its obstacles, including the power lines and eucalyptus groves, were well-known to the pilot.
- Pilot distraction was identified as the active causal factor.
- The investigation concluded that a combination of overconfidence, the completion of most tasks, and a loss of situational awareness led to a lapse in vigilance during the final maneuver.
