What happened
During the preflight phase, the pilot requested that the aircraft's fuel tanks be topped off, following an initial request for 100 lbs of fuel in each tank. During the subsequent inspection, the pilot decided additional fuel was necessary and reiterated the request to fill the tanks completely. However, the pilot failed to verify the actual fuel levels or inspect the gauges prior to departure.
Approximately one hour into the flight, the pilot checked the gauges and discovered that the tanks had not been filled as requested, with levels showing only 900 to 1,000 lbs per side. To extend the flight's duration, the pilot decreased engine power. As fuel levels dropped to approximately 400 to 500 lbs per side, the low fuel indicators for both wing tanks activated.
Upon realizing the fuel shortage, the pilot notified air traffic control and diverted to the closest available airport. During the visual approach, the pilot was traveling at a high speed and at an incorrect altitude. This resulted in a hard landing where the nose gear and left main landing gear collapsed. The aircraft subsequently veered off the runway, causing significant damage to the left wing.
