What happened
On December 7, 2025, at approximately 12:52 UTC, a Cessna A188B, registration HI799, was performing agricultural spraying operations at the Aeródromo de Boca de Mao in the Valverde Province, Dominican Republic. The aircraft was conducting its fifth flight of the day when the pilot noticed an obstruction in the left rudder pedal mechanism during a pre-landing check. The pilot immediately reported this mechanical issue to the company's maintenance department via radio.
Upon touchdown, the pilot experienced a loss of directional control due to the ineffective left pedal. This caused the aircraft to skid to the right, resulting in a runway excursion onto irregular terrain. During the excursion, the left landing gear collapsed and broke, and the propeller blades struck the ground. The pilot, who was the sole occupant, was uninjured and evacuated the aircraft on his own.
The investigation
The Dominican Republic's Aviation Accident Investigation Commission (CIAA) was notified of the accident on the day of the occurrence. An investigation team was formed to evaluate operational, human performance, airworthiness, and meteorological factors. The investigation was conducted in accordance with ICAO Annex 13 protocols, with the NTSB notified as the state of manufacture.
