Engine fire and overrun during takeoff

No fatalities • Santa Rosa de Yacuma, Bolivia • Takeoff (climb)

An aircraft experienced an engine explosion during its takeoff roll, leading to a runway overrun and subsequent fire.

What happened

While performing a takeoff roll, the aircraft's number two engine experienced an explosion and ignited a fire. This failure occurred before the aircraft reached its decision speed (V1). In response to the emergency, the crew aborted the takeoff attempt and applied emergency braking to stop the plane.

Despite these efforts, the aircraft was unable to halt before the end of the runway. The plane overran the paved surface and eventually came to a stop, at which point the fire spread, causing the aircraft to be destroyed. The flight was originally scheduled to travel to La Esperanza to collect a cargo of meat.

Findings

  • All four occupants of the aircraft managed to evacuate the wreckage without injury.
  • The engine explosion was the primary event that prevented the aircraft from completing the takeoff.

Probable cause

An explosion and subsequent fire in the number two engine during the takeoff roll led to a runway overrun.
