What happened
During the initial climb phase following departure from San Juan Airport, the crew of the aircraft reported that the number four engine had ignited. In response to this emergency, air traffic control authorized the flight to perform an immediate return to the airfield.
The aircraft executed a standard approach and successfully touched down on the runway. However, as the plane continued along the runway for approximately 950 meters, it lost directional control and drifted toward the right side of the pavement. As the aircraft transitioned onto soft terrain, the nose landing gear failed, causing the plane to come to a halt.
Despite the intensity of the incident, all four crew members managed to evacuate the aircraft without sustaining any injuries.
Findings
Investigations into the event determined that a fire originated in the fourth engine during the early stages of flight. The specific reason for the ignition of the engine remains undetermined.
