What happened
On 27 February 2011, a modified Grumman Goose with registration N221AG was prepared for a flight from Al Ain International Airport to Riyadh, Saudi Arabia. Prior to departure, a mechanic notified the Aerodrome Controller that the crew intended to perform a single test circuit and a low approach to verify the aircraft's condition following a period of inactivity. The aircraft, which featured turbine engine modifications, was also carrying an extra fuel bladder in the cabin.
During the pre-flight phase, the aircraft was fueled with 1,898 liters of Jet-A1. The crew, consisting of two pilots and two mechanics, completed various checklists and communications with Ground Movement Control. At 16:05:37, the aircraft received clearance for takeoff from Runway 19.
As the aircraft climbed to an altitude between 300 and 400 feet, it began a left turn at approximately 130 knots. During this maneuver, the aircraft entered an increasing rate of turn and began losing altitude. At 16:07:11, the aircraft struck the ground near Taxiway F. The impact caused the aircraft to come to a rest approximately 32 meters from the initial contact point. The impact and subsequent fire resulted in 4 fatalities.
Findings
- The aircraft was undergoing a test flight to ensure operational readiness prior to an international flight.
- The aircraft was equipped with non-standard turbine engine modifications.
- The aircraft entered a steepening left turn while losing altitude during the initial climb phase.
