What happened
On August 10, 2014, an Antonov AN-140-100, registered as EP-GPA, was performing a takeoff from runway 29L at Mehrabad International Airport in Tehran, Iran. The aircraft, operated by Sepahan Airlines, was in the process of lifting off when the number two engine experienced a shutdown approximately two seconds prior to rotation. Following this engine failure, the aircraft crashed in the vicinity of the airport.
The impact and subsequent post-crash fire resulted in the total destruction of the airframe. The accident resulted in 40 fatalities, including all 2 flight crewmembers, all 4 flight attendants, and 34 of the 40 passengers. A total of 8 passengers survived the event, though 11 passengers sustained serious injuries.
Prior to the accident, the aircraft had arrived at Tehran from Isfahan at approximately 03:30. During the pre-flight preparations, the pilot in command and dispatcher conducted weight and balance calculations using performance charts. Due to specific load limitations regarding the 15-degree flap setting, the load sheet was revised to a 10-degree flap position, and the maximum takeoff mass was recalculated accordingly.
Findings
Investigation into the flight preparations indicated that the crew had been provided with all necessary flight release documentation, including the flight plan, weather reports, and relevant NOTAMs. The primary factor identified in the sequence of events was the shutdown of the number two engine immediately preceding the lift-off phase.
