Runway overrun involving four-engine aircraft at Yakutsk-Intl Airport

No fatalities • Yakutsk-Intl (Platon Oyunsky), Russia • Landing (descent or approach)

A four-engine aircraft overran the runway at Yakutsk-Intl Airport following an engine thrust configuration error, resulting in the destruction of the aircraft and several injuries.

What happened

On a night flight departing from Moscow-Domodedovo Airport, a four-engine aircraft carrying 99 passengers and 10 crew members arrived at Yakutsk-Intl Airport. After a flight duration of approximately five hours and 42 minutes, the aircraft landed on runway 23. During the landing roll, the flight engineer incorrectly configured the reverse thrust, applying it only to engines number 2 and 3 while engines 1 and 4 remained at takeoff power.

As the aircraft maintained a speed between 265 and 270 km/h, the crew identified the thrust discrepancy with only 655 meters of runway remaining. In an attempt to stop the aircraft, the captain initiated emergency braking. This maneuver caused all tires to burst, leading the aircraft to overshoot the runway at 200 km/h. The aircraft subsequently lost its undercarriage and traveled an additional 397 meters beyond the runway end, where the airframe broke into three sections. All 109 people on board were evacuated safely, though six injuries were reported, including two serious cases. The aircraft was a total loss.

Findings

  • The primary cause of the overrun was the incorrect selection of reverse thrust on only two of the four engines.
  • The loss of braking capability was exacerbated by the bursting of all aircraft tires during the emergency deceleration attempt.

Probable cause

The aircraft overran the runway due to an error in reverse thrust deployment that left two engines at high power during the landing roll.
