What happened
On March 17, 2023, at 11:50 UTC, a Douglas DC-9-82, registration YV3499, operated by RUTAS AÉREAS DE VENEZUELA (RAVSA), departed from La Chinita International Airport in Maracaibo, Venezuela. The aircraft was on a scheduled flight to Tocumen International Airport in Panama with 88 people on board, including 79 passengers and 9 crew members.
During the initial climb phase at approximately 4,50 and feet, the crew reported a loud noise accompanied by vibrations and erratic indications from the left engine, which was identified as an engine stall. The pilot in command immediately implemented the Quick Reference Handbook (QRH) procedures, declared an emergency, and notified Air Traffic Services of the decision to return to the departure airport. The aircraft landed safely at 12:20 UTC. There were no fatalities or injuries, though the aircraft sustained substantial damage to the left engine.
The investigation
JIAAC investigators conducted a series of technical and operational examinations, including interviews with the pilot and first officer, as well as a review of crew licenses and medical certifications. The investigation also involved an audit of the aircraft's maintenance records, including airworthiness certificates and compliance with scheduled maintenance.
Physical inspections were performed on the engine, including a visual examination and a borescope inspection. Investigators documented damage to the low-pressure turbine blades. The investigation is ongoing, with plans to send the engine to a specialized maintenance organization to determine the full extent of the internal damage.
