What happened
On August 30, 2025, a Let L-410 UVP-LW, registration YV1515, operated by Aero Transportes del Caribe C.A., was performing a scheduled flight from Los Roques Airport (SVRS) to Maiquetia International Airport (SVMI). The aircraft was carrying two crew members and ten passengers.
While climbing through FL180, the crew observed an increase in Interstage Turbine Temperature (ITT) and fluctuations in RPM within the number two engine. In an attempt to stabilize the temperature, the pilot reduced power to 70% NG (Gas Generator Speed). However, a sudden secondary temperature spike occurred, followed immediately by an engine fire. The captain promptly executed the procedures outlined in the Quick Reference Handbook (QRH) to extinguish the fire. After confirming the fire was out, the crew shut down the number two engine and declared an emergency with Maiquetia Area Control Center. The aircraft continued to its destination and landed without further incident. All passengers and crew members were unharmed.
The investigation
The JIAAC conducted an investigation involving interviews with the flight crew and a review of several documents, including flight logs, maintenance records, and airworthiness certificates. Investigators also inspected the aircraft's external and internal conditions and examined the maintenance history provided by the maintenance organization (OMAC N° 693).
The engine involved in the incident, a Walter M601D, was removed and sent to the State of Manufacture for detailed inspection to determine the exact cause of the failure. The investigation also involved notifications to the United States as the State of Design and Manufacture.
