Aircraft accident during takeoff at Toluca-Licenciado Adolfo López Mateos

2 fatalities • Toluca, Mexico • Takeoff (climb)

An aircraft crashed into a field following a reverse thrust deployment during the takeoff roll at Toluca-Licenciado Adolfo López Mateos, resulting in two fatalities.

What happened

On the runway 15 at Toluca-Licenciado Adolfo López Mateos, an aircraft was performing its takeoff roll when a critical mechanical issue occurred. After the plane surpassed the V1 decision speed, a warning light illuminated on the cockpit panel, indicating that the left engine's reverse thrust had deployed. Following established operational protocols, the crew proceeded with the takeoff attempt.

Shortly after the aircraft rotated and entered the initial climb phase, it experienced a significant roll to the left. The aircraft subsequently impacted an open field, where the wreckage caught fire. The debris field was located approximately 200 metres beyond the end of the runway. The accident resulted in two fatalities, as both pilots were killed in the crash. The aircraft was completely destroyed.

Findings

  • The deployment of the left engine reverse thrust after passing V1 speed was the primary factor leading to the loss of control during the climb.

Probable cause

The unintended deployment of the left engine's reverse thrust during the takeoff roll caused an asymmetric thrust condition that led to a roll and subsequent crash.
