What happened
On September 9, 2009, a Yakovlev Yak-40, registration RA-87447, operated by Bugulma Aviation Enterprise, was scheduled to perform charter flight BGM 9401. The original flight plan, authorized by the airline's squadron commander and deputy general director, was to fly from Kazan to Begishevo and then to Aktobe (Aktau). The flight was initially scheduled to depart at 06:00 UTC.
The crew, consisting of a captain, first officer, flight engineer, and flight attendant, completed their preliminary flight preparations on September 7, 2009, at the Bugulma base airfield under the supervision of the squadron's deputy commander.
On the day of the flight, the crew underwent medical examinations at the Kazan airfield medical unit at 04:40 UTC. During the subsequent pre-flight preparations, which were overseen by the captain and monitored by the Kazan duty navigator, the client requested an urgent modification to the flight path and departure time. The new route was set for Kazan to Aktobe, with a revised departure time of 08:30 UTC. However, due to the client's failure to arrive, the departure was further delayed until 11:00 UTC.
Findings
- The crew did not undergo a secondary medical examination despite the interval between their initial medical clearance and the actual departure exceeding six hours.
