On July 6, 2023, an An-2T aircraft, registration RA-02241, was conducting forestry and emergency rescue work in the Kirov region. The flight was part of a contract with the Kirov Public State Specialized Autonomous Institution 'Kirov Aviation and Ground Forest Protection Base.' The crew, consisting of a captain, a second pilot, and a flight observer, was operating a route beginning and ending at the Kuchany settlement.
Prior to the flight, the crew performed pre-flight preparations at the airfield. In accordance with FAP-128 regulations, because no medical professional was available at the airfield to conduct examinations, the captain made the decision to authorize the crew for flight. The flight plan had been approved by the Regional Center of Air Traffic Management (RegTs ES ORVD) earlier that morning.
Technical investigations into the incident focused on the engine's constant speed governor and its drive mechanism. The investigation examined the condition of the constant speed governor Р 9СМ-2 and its associated drive components to determine if mechanical wear or malfunction contributed to the event.
Research conducted by the Interstate Aviation Committee revealed that the constant speed governor and the drive were in working condition without signs of excessive wear. However, the investigation identified a failure in a paronite gasket located between the flanges of the governor drive and the nose casing of the governor.
