On October 26, 2024, a Mi-2 helicopter operated by OOO "Vyatkaavia" was engaged in a medical mission for the Kirov Regional State Budgetary Healthcare Institution. The flight was requested by the Emergency Medical Station of the City of $Kirov$ to perform a sanitary flight to the urban-type settlement of Darovskoye.
The pilot in command, who was the Deputy General Director for flight operations, had recently arrived in Kirov by passenger train following a medical examination in Moscow. The pilot had begun his duty at 8:00 AM that morning after a schedule change. At approximately 2:00 PM, the pilot received the dispatch request and reviewed the weather forecasts before proceeding with the mission.
During the subsequent investigation of the aircraft, registration RA-15758, technical examinations were performed on several flight instruments. The investigation included the analysis of two AGK-47U attitude indicators, a USHV-1K main rotor pitch indicator, and two ITE tachometers.
Technical findings revealed that several instruments were in an unsatisfactory condition. Specifically, one AGK-47U attitude indicator was likely operational and indicating a left bank at the time of impact, while another instrument's readings could not be determined due to a 180-degree rotation of the scale and aircraft silhouette relative to the horizon. The ITE-2 tachometer showed readings of 1% and 23.5% at the time of examination.
