What happened
On 4 August 2000, a Piper PA-32-300, registration G-MCAR, was involved in an occurrence at RAF Henlow in Bedfordshire. During the sequence of events, the pilot made two distinct attempts to utilize the toe brakes.

The investigation established that a witness provided information regarding the engine start sequence, correcting earlier phrasing that attributed the recollection directly to the pilot.
