Engine failure during takeoff leads to fatal crash in Baranikovskiy

1 fatality • Baranikovskiy, Russia • Takeoff (climb)

A single-engine aircraft crashed during a crop spraying mission in Baranikovskiy after experiencing engine power loss during a turn.

What happened

During a mission to spray rice paddy fields in Baranikovskiy, a single-engine aircraft experienced a catastrophic engine failure shortly after takeoff. Prior to the flight, the aircraft's fuel tank was replenished with 250 litres of E95 fuel. Although a technician noted that the fuel exhibited an unusual yellow color, the pilot chose to proceed with the mission.

In violation of established flight procedures, the pilot replaced the designated copilot with the owner of the area intended for treatment before departing the ground. During the takeoff roll, the aircraft failed to reach its expected acceleration, yet the pilot continued the ascent. At an altitude of approximately 30 metres, the pilot attempted a 90-degree turn to the left, at which point the engine lost all power.

In an effort to manage the situation, the pilot began discharging the chemical load and attempted an emergency landing. The aircraft struck the ground and traveled for roughly 134 metres before colliding with an irrigation drain. The impact caused the aircraft to nose over and ignite. The accident resulted in one fatality and one serious injury.

Findings

  • The pilot operated the flight despite being aware of abnormally colored fuel.
  • The crew composition was non-compliant with published procedures due to the unauthorized replacement of the copilot.
  • The engine lost power during a climbing turn following an inadequate takeoff acceleration.

Probable cause

The engine lost power during a climbing turn, potentially exacerbated by the use of contaminated fuel and unauthorized crew changes.
