What happened
During an ascent following departure from Salisbury-Wicomico County Regional Airport, the pilot experienced a simultaneous loss of power in both engines. In an effort to manage the emergency, the crew attempted to execute an unplanned return to the airfield for an emergency landing. During the final approach, while the aircraft was maneuvering back toward the runway with the left throttle set to idle, the plane entered a stall. The aircraft type impacted the ground approximately half a mile before reaching the runway. The impact resulted in the total destruction of the airframe and five fatalities.
Findings
Investigations into the accident identified several critical issues regarding fuel quality and maintenance oversight. The primary cause was determined to be a double engine failure resulting from improper servicing by ground personnel. Specific findings included:
- Presence of water contamination within the fuel supply
- Use of an incorrect fuel grade, specifically kerosene mixed with 100LL
- Inadequate training and supervision of ramp crews by operational management
- Failure to adhere to established emergency and operational procedures
- Challenges posed by high obstructions in the vicinity
