Fatal Stall Accident Involving Piper PA28R-200 Near Rand Aerodrome

Casualties unknown • Next to Germiston Golf Course in a stream, ZA

A private flight ended in tragedy when a Piper PA28R-200 stalled shortly after takeoff, resulting in three fatalities near a Johannesburg golf course.

What happened

On 24 August 2008, a Piper PA28R-200, registration ZS-MHA, departed Rand Aerodrome on a private VFR flight. During the initial climb, witnesses observed the aircraft flying at a low altitude and slow airspeed. As the aircraft passed over a nearby golf course, it lost altitude and the left wing struck trees, causing the wing to separate from the fuselage. The aircraft subsequently crashed into a shallow stream.

The impact resulted in 3 fatalities. A rear-seat passenger died at the scene, trapped beneath the wreckage. The pilot and the front-seat passenger, both of whom were transported to a hospital, succumbed to their injuries later that week.

Probable cause

The aircraft entered an unrecoverable stall because the pilot initiated rotation at an insufficient distance along the runway, failing to account for the increased takeoff requirements imposed by high-density altitude.
