11 fatalitiesMoscow-Sheremetyevo, RussiaTakeoff (climb)

An aircraft carrying eleven people crashed into a wooded area during a test flight following maintenance work, resulting in no survivors.

What happened

During a scheduled test flight following recent maintenance procedures, an aircraft occupied by five crew members and six technicians experienced a catastrophic loss of control. The flight commenced with a standard takeoff roll on runway 25, covering approximately 1,550 to 1,600 meters. After the pilot successfully rotated the aircraft and established a normal initial climb, the flight progressed normally for about 800 meters.

Upon reaching an altitude of 175 meters, the aircraft entered a sharp left bank at a 90-degree angle. The plane subsequently became uncontrollable and impacted a wooded area located near the airport, resulting in a massive explosion. The impact caused the total destruction of the aircraft type and led to 11 fatalities among all occupants on board.

Findings

Investigations into the accident revealed that the crash was the result of a critical error made during the maintenance phase. It was determined that technicians had incorrectly reconnected the rudder control system, failing to follow the manufacturer's established procedures. This mistake caused the rudder control system to be cross-connected, which rendered the aircraft impossible to steer properly during the climb.

Probable cause

The accident was caused by a maintenance error where the rudder control system was incorrectly cross-connected during post-maintenance work.
