The airplane departed Kholmskaya on a supply mission to Gelendzhik, carrying ammunition. 2-3 minutes after takeoff, it was shot down by a German fighter and crashed. The pilot was killed. Probable cause (official findings): Shot down by a German fighter.
- August 21, 1935AccidentHull loss
Upon landing, the aircraft went out of control and came to rest. The pilot was unhurt and the aircraft was damaged beyond repair.
- February 14, 1921Accident
En route from Cheyenne to Salt Lake City, the oil pressure dropped to 9 or 10. The engine temperature increased to 192° and the revolutions per minute dropped to 1,200. The plane crashed on a mountain side near Telephone Canyon. The pilot was injured.
