The crew was completing a local training sortie in Rio de Janeiro when the airplane crashed in unknown circumstances in the Bay of Guanabara. A crew member was killed and the second was injured.

Upon landing, the engine failed. The aircraft overturned and came to rest upside down. Both occupants were injured. Probable cause (official findings): Engine failure upon landing.
- July 1, 1920AccidentHull loss
Following a local training mission, the pilot was returning to Kalijati Airfield when, upon landing, the airplane crashed and came to rest upside down. The pilot escaped uninjured and the aircraft was damaged beyond repair.
