- April 4, 1991Accident
The aircraft was completing a charter flight from Sturdee Valley to Smithers on behalf of a mine company. While flying at low height above Thutade Lake in poor weather conditions, a wing struck the iced surface of the lake and the aircraft crashed. A passenger was rescued while six other occupants were killed.

A99A0101: On landing, a bolt pulled through a supporting gear strut, and the gear collapsed. The aircraft skidded about 130 feet before nosing over and coming to a stop. The pilot and eleven year old passenger were uninjured. The aircraft had logged about two hundred previous landings.
