An incorrectly seated and misaligned hydro-mechanical unit resulted in significant wear of the hydro-mechanical unit drive shaft and accessory gearbox internal shouldered shaft. This resulted in the hydro-mechanical unit slipping and decoupling from the engine accessory gearbox. Full accident record
