Few minutes after takeoff from RAF Madley, the twin engine airplane collided with a RAF Percival Proctor. Registered DX223, it was carrying two people. Following the collision, both airplane dove into the ground and crashed in Lugwardine. All nine people in both airplanes were killed. Crew: Cpl Robert Moss, Ac1 Ronald Perry, Ac2 Cyril Thomas Clinnick, Ac2 John Philip Exten, Ac2 William Potts, Ac2 Roy Albert Sherwin, Ac2 Graham Eugene Stevenson.
