Alaska Cessna 441 crash probe weighs visibility and terrain risks

US investigators have yet to determine why a Cessna 441 struck terrain in Alaska, but preliminary findings flag low visibility and an approach plate warning that a late go-around was improbable.

Investigators have yet to establish what caused a fatal Cessna 441 terrain collision in Alaska. All eight occupants died. Preliminary findings point to low visibility and warnings about late go-arounds.

The aircraft was bound for Cape Newenham airport on 20 August. It was on its second approach to runway 15 when it went down. The crew had aborted the first attempt, though it is unclear when that approach was called off, because ADS-B coverage of the aircraft's position was intermittent.

A July 2026 approach plate for the airport shows the missed approach point, CFKMW, about 2.2 statute miles from the runway 15 threshold. It notes high terrain on both sides of the airport and near the southern end of the runway. A successful go-around is improbable if started past the missed approach point, it warns.

Shortly after the abort, the crew told an air traffic controller they were in visual conditions. Observations from ground personnel indicate visibility later worsened. According to the National Transportation Safety Board, the crew considered diverting to Platinum airport but could not reach it by radio, and were cleared for a second approach to Cape Newenham. That approach was also flown to runway 15 using area navigation. No further radio communications came from the flight crew, the inquiry said.

The airport's weather observation system reported a quarter-mile visibility, fog, and an overcast ceiling at 300ft. The approach plate lists a minimum descent altitude of 1,320ft with 3sm visibility. The aircraft crossed CFKMW at about 1,000ft and continued beyond it, descending further.
