- 2025-04 B747-400 flight crew reported a windshield crack during cruise. The QRH procedures were completed and the flight diverted to a suitable airport where a safe landing was completed.
- 2024-04 B747-400 flight crew reported during approach they had a #1 engine failure which caused the aircraft to turn left. They corrected and were stable on the localizer and landed without incident.
- 2023-08 Air carrier Captain reported a sharp acrid odor during cruise. After conferring with Dispatch the odor was determined to be caused by a volcanic ash event which was remedied by climbing to a higher altitude.
