- 2015-12 SCT Controller reports that hundreds of incidents of radar targets jumping away from the aircraft's actual position on the radar display have occurred since the installation of new STARS ATC processing software. Other facilities using this software have also reported the same problem.
- 2015-12 Controller on the Job training was occurring at the Local Control position. The trainee Controller cleared an aircraft for takeoff with another aircraft on short final to an intersecting runway. The instructor took over and sent the arrival around preventing a loss of separation. The reporter was Controller-in-Charge at the time and felt the instructor should have resolved the situation in a timelier manner.
- 2015-12 B737 Captain lamented an ATIS message that was an hour and 45 minutes old and not current during arrival to MDW. He believed that all Tower personnel will need to become familiar with the new AC's relating to the new Runway Condition Assessment Matrix (RCAM).
