AIRCRAFT 2/C402 WAS CLEARED TO LAND RUNWAY 22R. AIRCRAFT 1 WAS ISSUED LEFT TURN ON TAXIWAY N. THE PILOT READBACK WAS CORRECT. AIRCRAFT 1 MISSED THE TURN ONTO TAXIWAY N AND ENTERED RUNWAY 15R AND RUNWAY 22R INTERSECTION WITHOUT APPROVAL. AIRCRAFT 2 ON LANDING ROLLOUT CROSSED RUNWAY 15L AT 70KTS. PILOT READ BACK WAS CORRECT. AIRCRAFT 1 ENTERED RUNWAY 9 AND THEN DEPARTED WITHOUT CLEARANCE. AIRCRAFT 1 PASSED TAXIWAY K AS AIRCRAFT 2 APPROACHED THE RUNWAY 4R DISPLACED THRESHOLD. AIRCRAFT 2 WAS SENT AROUND AS AIRCRAFT 1 WENT THROUGH THE RUNWAY INTERSECTION. ASDE-X ALERTED, AND RUNWAY 9 STATUS LIGHTS DISPLAYED RED. THE CLOSEST ESTIMATED PROXIMITY IS 331 FEET LATERAL. AN OVERFLY DID NOT OCCUR.

AIRCRAFT 1 (E190) WAS INSTRUCTED TO CROSS RUNWAY 15R AT SAME TIME AIRCRAFT 2 (A321) WAS CLEARED AND ON TAKE-OFF ROLL FOR SAME RUNWAY. AIRCRAFT 1 LANDED RUNWAY 4R AND WAS INSTRUCTED BY THE LOCAL CONTROL WEST (LCW) CONTROLLER TO TAXI VIA RUNWAY 33R, CROSS RUNWAY 4L, AND HOLD-SHORT OF RUNWAY 15R. AIRCRAFT 1 READ-BACK WAS CORRECT. AIRCRAFT 2 WAS TAXI ON TAXIWAY LIMA AND GIVEN INSTRUCTION BY THE LCW CONTROLLER TO LUAW ON RUNWAY 15R. AS AIRCRAFT 1 WAS COMING TO A STOP ON TAXIWAY NOVEMBER FOR THE HOLD-SHORT LINE, THE PILOT CHECKED IN WITH GROUND AND WAS INSTRUCTED BY THE GROUND CONTROL (GC1) CONTROLLER TO CROSS RUNWAY 15R. FIVE SECONDS AFTER AIRCRAFT 1 WAS GIVEN CLEARANCE TO CROSS RUNWAY 15R, THE LCW CONTROLLER CLEARED AIRCRAFT 2 FOR TAKE-OFF FROM RUNWAY 15R. AS AIRCRAFT 2 WAS BEGINNING DEPARTURE ROLL, THE BOS LOCAL CONTROL EAST (LCE) CONTROLLER NOTICED THE DEVELOPING CONFLICT AND ALERTED THE LCW AND GC1 CONTROLLERS. THE LCW CONTROLLER INSTRUCTED AIRCRAFT 1 AND AIRCRAFT 2 TO STOP. AIRCRAFT 2 ADVISED THEY WERE REJECTING. THE GC1 CONTROLLER ALSO INSTRUCTED AIRCRAFT 1 TO STOP, WITH NO RESPONSE FROM AIRCRAFT 1. THE ASDE-X DID ALERT; HOWEVER, LCE CONTROLLER RECOGNIZED THE CONFLICT JUST PRIOR THE ALERT. ASDE ANALYSIS INDICATED AIRCRAFT 2 WAS TRAVELING AT A SPEED OF 116KNTS AND 700FT FROM THE INTERSECTION OF RUNWAY 15R AND TAXIWAY NOVEMBER WHEN ABORTING THE TAKE-OFF, AIRCRAFT 1 WAS TRAVELING 11KNTS AS THE AIRCRAFT ENTERED ONTO THE RUNWAY. BOTH AIRCRAFT CAME TO A COMPLETE STOP WITH THE SEPARATION BETWEEN AIRCRAFT 1 AND AIRCRAFT 2 AT APPROXIMATELY 266FT. AIRCRAFT 2 REQUESTED RIGHT TURN ONTO TAXIWAY NOVEMBER AND A RIGHT ONTO TAXIWAY BRAVO. ONCE AIRCRAFT 2 WAS CLEAR OF TAXIWAY NOVEMBER, AIRCRAFT 1 WAS THEN CLEARED TO CROSS RUNWAY 15R. VALIDATED A LOSS OF SEPARATION.

AN AIRBUS A333/H WAS CLEARED FOR TAKEOFF RUNWAY 15R. A BOEING B737 WAS CLEARED FOR TAKEOFF ON INTERSECTING RUNWAY 9. THE A333 WAS AIRBORNE AND APPROXIMATELY 300 FEET VERTICAL AT THE 15R/9 INTERSECTION WHEN THE B737 WAS ROLLING DOWN RUNWAY 9 AND APPROXIMATELY 1,000 FEET FROM THE 9/15R INTERSECTION. THE B737 CONTINUED AND DEPARTED. AS PER A DOCUMENT WITH ASR DATA OVERLAPPING AN AIRPORT DIAGRAM THE A333 WAS STILL ON THE GROUND ABEAM TAXIWAY DELTA AND DID NOT SHOW 300 AS AN ALTITUDE UNTIL 5 SECONDS LATER, EQUAL DISTANCE FROM DELTA ON THE OTHER SIDE OF THE 9/15R INTERSECTION. THE B737 WAS STILL ON THE GROUND ABEAM TAXIWAY D-1, PAST THE 9/15R INTERSECTION. AMASS DATA INDICATES THAT THE B737 HAD A VELOCITY OF 245 FT/S WITH AN ACCELERATION OF 6 FT/S^2 WHEN THEY WERE ABOUT 171FT FROM THE 9/15R INTERSECTION OF THE TWO RUNWAYS. THE A333 HAD A VELOCITY OF 291 FT/S WITH AN ACCELERATION OF 1 FT/S^2, AS THEY WERE JUST EXITING THE 9/15R INTERSECTION.
