JS31 WAS ISSUED TAXI INSTRUCTIONS FOR A RUNWAY 36R DEPARTURE AND WAS TOLD TO HOLD SHORT OF RUNWAY 36R AT TAXIWAY DELTA 3. B350 WAS ISSUED TAXI INSTRUCTIONS FOR A RUNWAY 36C DEPARTURE AND WAS INSTRUCTED TO HOLD SHORT OF RUNWAY 36R AT TAXIWAY ROMEO. B734 WAS LANDING RUNWAY 36R. AS THE B734 PASSED TAXIWAY DELTA 3, GROUND CONTROL (GC) INTENDED TO CLEAR THE JS31 ACROSS RUNWAY 36R BEHIND THE LANDING B734 BUT MISTAKENLY ISSUED A CLEARANCE FOR THE B350 TO CROSS RUNWAY 36R WITHOUT DELAY. GC REALIZED THE MISTAKE AND IMMEDIATELY INSTRUCTED THE B350 TO HOLD SHORT OF THE RUNWAY. B350 WAS ACROSS THE HOLD SHORT LINE AS B734 PASSED TAXIWAY ROMEO. B734 CALLED THE TOWER AND STATED THAT THE B350, “ALMOST HIT US.” THE ASDE-X DID NOT ALARM.

GROUND CONTROL EAST (GC-E) RECEIVED APPROVAL FROM LOCAL CONTROL EAST (LC-E) TO CROSS RUNWAY 18L AT TAXIWAY MIKE FOR /CRJ7, ASSIGNED RUNWAY 18L FOR DEPARTURE VIA TAXIWAYS MIKE AND DELTA. BUT AS THE CRJ7 BEGAN CROSSING, LC-E ISSUED A TAKEOFF CLEARANCE TO CRJ2 THAT WAS IN POSITION ON THE RUNWAY 18L NUMBERS. THE ASDE-X ALERTED THE CONFLICT, BUT THE LC-E CONTROLLER DETECTED THAT THE CRJ2 HAD ALREADY INITIATED THE ABORTED TAKEOFF AND DID NOT CANCEL THE TAKEOFF CLEARANCE. THE CRJ2 PILOT INDICATED THAT HE HAD REACHED 40-50 KNOTS AT THE TIME OF THE REJECTED TAKEOFF AND DID NOT CONSIDER HIS ACTION TO BE A HIGH SPEED ABORT. THE CLOSEST PROXIMITY BETWEEN THE 2 AIRCRAFT WAS CALCULATED TO BE 414 FEET ON THE RUNWAY SURFACE. SUBSEQUENTLY, THE CRJ2 TAXIED BACK TO RUNWAY 18L VIA A RIGHT TURN ONTO RUNWAY 5, FOLLOWED BY TAXIWAYS ROMEO, MIKE AND CHARLIE.

CLT WAS IN A SOUTH OPERATION LANDING RUNWAYS 18C AND 23. LOCAL CONTROL EAST (LCE) CLEARED A CANADAIR CRJ2 FOR TAKEOFF ON RUNWAY 18L FULL LENGTH. A PILATUS PC12 WAS WAITING TO GO NEXT ON RUNWAY 18L AT INTERSECTION ALPHA. GROUND CONTROL HAD WRITTEN ON THE STRIP AND VERBALLY COORDINATED THAT THE PC12 WAS REQUESTING AN INTERSECTION DEPARTURE OFF 18L AT ALPHA. LCE THEN CLEARED THE PC12 TO TAXI INTO POSITION AND HOLD (TIPH) ONTO RUNWAY 18L NOT REALIZING THE AIRCRAFT WAS AT AN INTERSECTION DOWNFIELD (DISTANCE FROM APPROACH END 18L TO ALPHA IS APPROXIMATELY 2,600 FEET). THE CRJ2 HAD ALREADY BEGUN DEPARTURE ROLL AND HAD ROLLED APPROXIMATELY 1,600 FEET AS THE PC12 PROCEEDED ONTO THE RUNWAY. ASDE-X ALERTED. THE CRJ2 PILOT BEGAN TO BREAK THE AIRCRAFT AND LC2 CANCELLED THE TAKEOFF CLEARANCE. THE PC12 PILOT HAD PULLED TO THE LEFT SIDE OF THE RUNWAY WHEN HE RECOGNIZED WHAT WAS HAPPENING AND THE CRJ2 STOPPED APPROXIMATELY 10 FEET FROM THE PC12. CLOSEST PROXIMITY ON THE SURFACE REPORTED WAS TEN (10) FEET HORIZONTAL. ASDE-X ANALYSIS INDICATED THE CRJ2 WAS IN THE INTERSECTION OF 18L/23 AT AN APPROXIMATE SPEED OF 227 FEET PER SEC (ESTIMATED 480 FEET FROM ALPHA) WHEN THE PC12 WAS ENTERING THE RUNWAY. CLOSEST PROXIMITY WAS ESTIMATED AT 25 FEET.
