VEHICLE CROSSED RWY 8L WITHOUT ATC AUTHORIZATION. AIRCRAFT 2 WAS ON LANDING ROLL RWY 8L AND ASKED LC IF THEY "SAW THE TRUCK CROSS UNDERNEATH THEM". LC CALLED VEHICLE AND VEHICLE STATED THAT THEY CROSSED RWY 8L AT TWY A SOUTHBOUND AS AIRCRAFT 2 WAS LANDING. AIRCRAFT 2 ALTITUDE WAS LESS THAN 100FT WHEN THEY OVERFLEW VEHICLE. FACILITY REPORTED ASDE ALERTED AS AIRCRAFT 2 WAS JUST SHORT OF THE THRESHOLD.

THE LOCAL CONTROLLER INSTRUCTED AIRCRAFT 1 TO EXIT THE RUNWAY TO THE RIGHT ON TAXIWAY G AND TURN LEFT ON TAXIWAY B. THE PILOT READ BACK WAS CORRECT. THE LOCAL CONTROLLER THEN INSTRUCTED AIRCRAFT 1 TO TAXI VIA B, E, HOLD SHORT OF RUNWAY 4L AT TAXIWAY E. THE PILOT READ BACK WAS CORRECT. AIRCRAFT 1 VERIFIED TO TURN RIGHT ON E. LC VERIFIED TURN RIGHT ON E HOLD SHORT OF RUNWAY 4L. THE PILOT READ BACK WAS CORRECT. THE LOCAL CONTROLLER CLEARED AIRCRAFT 2/BE9L FOR TAKEOFF ON RUNWAY 4L. AS AIRCRAFT 1 BEGAN CROSSING THE RUNWAY 4L HOLD BAR, THE LOCAL CONTROLLER VERIFIED AIRCRAFT 1 WAS HOLDING SHORT OF RUNWAY 4L. THE PILOT RESPONSE WAS UNINTELLIGIBLE. LC INSTRUCTED AIRCRAFT 1 TO STOP. AIRCRAFT 1 ENTERED RUNWAY 4L AT TAXIWAY E AS AIRCRAFT 2 WAS PASSING THROUGH THE RUNWAY 4L/TAXIWAY E INTERSECTION ON DEPARTURE ROLE. ASDE-X REPLAY ESTIMATES CLOSEST PROXIMITY AT 54 FEET OR LESS LATERALLY.

MD520 (HELO) WAS LOCATED ON THE SOUTH RAMP IN BETWEEN TAXIWAYS ECHO AND PAPA AND CALLED LOCAL CONTROL (LC) FOR A WEST LOCH DEPARTURE (WLD) AND REQUESTED A TURN TO THE EAST WHEN ABLE. WLD IS A VFR DEPARTURE PROCEDURE THAT ROUTES AIRCRAFT TO THE NORTHWEST OF HNL BEFORE TURNING ON COURSE. LC CONTROLLER CLEARED THE MD520 TO DEPART DIRECTLY TO THE EAST WITH NO DEPARTURE RESTRICTIONS. MD520 DID NOT HEAR THE FULL CLEARANCE AND REQUESTED THE LC CONTROLLER TO REPEAT. LC CONTROLLER THEN CLEARED THE MD520 DEPARTURE DIRECT TO PALI. MD520 DEPARTED THE SOUTH RAMP NORTHWEST BOUND INTO THE WIND BEFORE TURNING DIRECT PALI. MD520 OVERFLEW PA23 ON LANDING ROLL ON RUNWAY 22L BY AND ESTIMATED 95 FEET.

C208 VFR INBOUND TO RUNWAY 4L REQUESTED RUNWAY 8L. PILOT WAS ISSUED LANDING CLEARANCE ON RUNWAY 4L AND THE PILOT READ-BACK WAS CORRECT.. PILOT WAS RE-CLEARED TO LAND ON RUNWAY 4L WITH CORRECT PILOT ACKNOWLEDGEMENT. ASDE-X ALERT ALARMED, WARNING RUNWAY 4R CLOSED, AS THE AIRCRAFT WAS ROLLING OUT ON RUNWAY 4R WHICH WAS CLOSED FOR MAINTENANCE AND OCCUPIED BY PERSONNEL AND EQUIPMENT. AIRCRAFT CAME TO A STOP APPROXIMATELY 1000 FEET FROM THE APPROACH END OF RUNWAY 4R AND APPROXIMATELY 100 FEET FROM AN ARRESTING BARRIER WITH ASSOCIATED PERSONNEL AND EQUIPMENT ON AND ADJACENT TO THE RUNWAY. PILOT STATED HE HAD LOST SITUATIONAL AWARENESS TURNING LEFT BASE AND LANDED ON THE WRONG RUNWAY. LOSS OF SURFACE SEPARATION DUE TO VEHICLES AND PERSONNEL ON CLOSED RUNWAY 4R AS AIRCRAFT LANDED.

DISTRICT 4, AN AIRPORT VEHICLE, WAS CONDUCTING RUNWAY CHECKS AND WAS PROCEEDING TO RUNWAY 8R ON TAXIWAY ROMEO BRAVO BUT EXECUTED AN UNANNOUNCED TURN BACK TOWARD RUNWAY 8L. B767 WAS SENT AROUND ON SHORT FINAL FOR RUNWAY 8L AT APPROXIMATELY 500 FEET FROM THRESHOLD AFTER THE ASDE ALARMED AROUND DUE TO THE VEHICLE ENTERING THE APPROACH END OF RUNWAY 8L WITHOUT A CLEARANCE. ATTEMPTS BY GROUND CONTROL (GC) TO COMMUNICATE WITH DISTRICT 4 WERE UNSUCCESSFUL. B767 PASSED OVERHEAD THE VEHICLE AT A LOW ALTITUDE (ALTITUDE UNKNOWN PENDING ADDITIONAL DATA). THIS EVENT OCCURRED IN AN AREA IDENTIFIED AS A “HOT SPOT”.

AN F15 LANDED RUNWAY 4R AND WAS INSTRUCTED TO TURN LEFT ON TAXIWAY KILO, CROSS RUNWAY 8L. THE F15 PILOT READ BACK THE INSTRUCTIONS. APPROXIMATELY 2 MINUTES AFTER THE F15 HAD LANDED A BOEING B767 LANDED ON RUNWAY 4R. THE B767 PILOT REPORTED AN AIRCRAFT ON THE RUNWAY AHEAD OF THEM. LOCAL CONTROL DID NOT ENSURE THE RUNWAY WAS CLEAR AND CLEARED THE B767 TO LAND. BROADCAST WAS MADE IN THE BLIND AND IT WAS DETERMINED TO BE THE F15 THAT WAS INSTRUCTED TO EXIT AT KILO AND TO CROSS RUNWAY 8L. THE B767 PILOT STATED THE FIRST THEY SAW OF THE F15 WAS THE TAILPIPES AND THEY BRAKED HARD AT ABOUT 70-80 KNOTS AND WHEN THEY STOPPED THEY WERE LOOKING DOWN ON THE F15. B767 PILOT STATED THE F15 WAS AT THE HOLDING POSITION MARKING ON RUNWAY 4R SHORT OF RUNWAY 8L AND ORIENTED DOWN RUNWAY 4R. THE LAHSO LIGHTS ON 4R SHORT OF 8L ADDED TO THE DIFFICULTY OF SEEING THE F15. CLOSEST HORIZONTAL PROXIMITY WAS LESS THAN 200 FEET. F15 PILOT REPORTED CANOPY WAS FOGGED.

A PIPER PA27 WAS CLEARED TO LAND RUNWAY 4R, WHICH PILOT ACKNOWLEDGED. LOCAL CONTROL (LC) HAD ALSO CLEARED A CESSNA C172 FOR DEPARTURE RUNWAY 4L WHICH IS OFFSET FROM RUNWAY 4R APPROXIMATELY 2,050 FEET DOWN FROM RUNWAY 4R THRESHOLD. WHEN THE PA27 WAS APPROXIMATELY 1 TO 1.5 MILE FINAL, BOTH LC AND GC REALIZED THE PA27 APPEARED TO NOT BE LINED UP FOR RUNWAY 4R. LC TRIED TWICE TO CALL THE PA27 PILOT AND BOTH TIMES USED THE WRONG CALL SIGN. AFTER THE SECOND ATTEMPT THE PA27 PILOT TRANSMITTED "ARE YOU TRYING TO GET ME?" LC REPLIED YES AND SENT THE PA27 AROUND. THE PA27 HAD ALREADY FLEW OVER THE C172 AT APPROACH END RUNWAY 4L WHEN THE PA27 PILOT ACKNOWLEDGED THE GO AROUND. THE PA27 FLEW OVER THE C172 BY AN ESTIMATED 190 FEET BY ATCT BASED ON TOWER HEIGHT AND AS THE PA27 AIRCRAFT FLEW BY. THE C172 HAD NOT BEGUN TAKE-OFF ROLL BECAUSE HE HEARD THE TRANSMISSIONS BY LC. THE PA27 STARTED HIS CLIMB IN THE VICINITY OF TAXIWAY DELTA, APPROXIMATELY 1,592 FEET FROM THE APPROACH END OF 4L.

A CESSNA C172 IS CLEARED TO LAND RUNWAY 22L. APPROXIMATELY 3 MINUTES LATER ANOTHER C172 CALLS READY FOR TAKEOFF RUNWAY 22L AT TAXIWAY PAPA. LOCAL CONTROL 1 (LC1) INSTRUCTS THE C172 AT PAPA "RUNWAY 22L POSITION AND HOLD", THERE WAS A SHORT PAUSE, THEN LC1 CONTINUES "I TELL YOU NO HOLD SHORT RUNWAY 22L HAVE TO HOLD YOU FOR WAKE TURBULENCE". THE C172 RESPONDS "RUNWAY 22L POSITION AND HOLD" THEN PROCEEDS INTO POSITION WHEN THE LANDING C172 IS TOUCHING DOWN RUNWAY 22L ON THE NUMBERS 2,750 FEET FROM PAPA. THE LANDING C172 STOPPED NORMALLY AT THE INTERSECTION OF RUNWAY 26R AND 22L, 400 FEET FROM PAPA. THE LANDING C172 DID NOT SEE THE OTHER C172 HOLDING ON THE RUNWAY.
