LC THEN INSTRUCTED VEHICLE/MAINTENANCE FLIGHT TO CROSS RWY 13L AND RWY 13R ON TWY K. VEHICLE CROSSED RWY 13L AND ENTERED RWY 13R AT TWY K AS AIRCRAFT 2 WAS PASSING TWY H. THE ASDE ALERTED AND LC INSTRUCTED VEHICLE TO HOLD SHORT OF RWY 13R, BUT THE AIRCRAFT WAS ALREADY IN THE MIDDLE OF THE RUNWAY. VEHICLE CLEARED RWY 13R AT TWY K PRIOR TO AIRCRAFT 2 REACHING THE INTERSECTION. AIRCRAFT 2 ADVISED LC THAT THEY WERE REJECTING THEIR TAKEOFF AND EXITING THE RUNWAY AT TWY L. THE CLOSEST PROXIMITY FROM THE ASDE ANALYSIS WAS 176 FEET LATERAL.

AIRCRAFT 1 ENTERED RUNWAY 22 AND BEGAN DEPARTURE ROLL WITHOUT ATC AUTHORIZATION WHEN AIRCRAFT 2 WAS ON AN APPROXIMATE 1/4 MILE FINAL. WHEN AIRCRAFT 1 WAS APPROXIMATELY 1975 FEET DOWN RUNWAY 22, LC INSTRUCTED THEM TO STOP. AIRCRAFT 1 CONTINUED DEPARTURE AND BOTH AIRCRAFT WENT THROUGH THE RUNWAY 22/RUNWAY 13R INTERSECTION AT THE SAME TIME. AIRCRAFT 1 LIFTED OFF AND REPORTED TO LC THAT THEY HAD HIT AIRCRAFT 2. THE WING OF AIRCRAFT 1 HIT THE TAIL OF AIRCRAFT 2.

    AIRCRAFT 1, C208 DEPARTED RUNWAY 22 WITH 2 VEHICLES ON THE RUNWAY. THE VEHICLE WAS HOLDING AT TAXIWAY KILO 1 AND REQUESTED TO ENTER RUNWAY 4 TO WORK ON RUNWAY LIGHTS BETWEEN THE APPROACH END AND RUNWAY 13R. LOCAL CONTROL (LC) INSTRUCTED THE VEHICLES TO ENTER RUNWAY 4 AND HOLD SHORT OF RUNWAY 13R. THE VEHICLES ENTERED THE RUNWAY AND BEGAN WORKING. AIRCRAFT 1 CALLED FOR TAXI AND THE CONTROLLER INSTRUCTED AIRCRAFT 1 TO TAXI VIA TAXIWAY ROMEO FOR A RUNWAY 22 AT TAXIWAY ROMEO DEPARTURE. THE CONTROLLER THEN ISSUED A TAKEOFF CLEARANCE TO AIRCRAFT 1 AS VEHICLE WAS STILL ON THE RUNWAY. AIRCRAFT 1 DEPARTED RUNWAY 22 AND OVERFLEW THE VEHICLES ON THE RUNWAY. THE VEHICLE DRIVER SPOKE OUT OVER THE FREQUENCY AND INQUIRED ABOUT WHAT HAPPENED. THE ASDE DID ALERT. THE CLOSEST PROXIMITY ACCORDING TO THE ASDE ANALYSIS WAS 90 FEET HORIZONTAL AND 75 FEET VERTICAL.

    PD/OI RI WITH CONFLICT. THE FINAL CONTROLLER CLEARED A/C 1 FOR A VISUAL APPROACH RY 12L AFTER COORDINATION THAT STATED IF THE A/C WAS PARKING AT SIGNATURE RY 12L WAS BETTER. THE FINAL CONTROLLER ALSO PUT IT IN THE SCRATCHPAD AS PER DIRECTIVES. THE LOCAL CONTROLLER CLEARED THE LJ35 TO LAND ON RY 12R BUT THE PILOT DID NOT READ BACK THE RUNWAY. AS A/C 2 IS CROSSING RY 12L THE LJ35 IS ON SHORT FINAL FOR THAT RUNWAY. ASDE-X ALARMED.. LR35 PILOT STATED THAT HE SAW THE B737 APPROACHING RY 12L AT A HIGH RATE OF SPEED AND DECIDED TO INITIATE A GO AROUND AT APPROXIMATELY 500 FEET. ACCORDING TO THE QCG PAGE OUT CLOSEST PROXIMITY WAS 147 FEET LATERAL, AND 100 FEET VERTICAL. A/C1 APPEARED TO OVERFLY A/C 2 ON ASDE-X REPLAY.

    LOCAL (LC) CLEARED A BOEING B737 FOR DEPARTURE ON RUNWAY 12R FULL LENGTH AND THEN CLEARED A BELL 06 (206 HELICOPTER) VFR FOR NORTH DEPARTURE (COUGAR DEPARTURE) FROM THE SOUTH RAMP HELICOPTER PAD WITH NO RESTRICTIONS. NORMALLY HELICOPTERS DEPART TOWARDS THE ARRIVAL END OF RUNWAY 4 AND TURN NORTH OVER MID-FIELD AND CONTINUE NORTHBOUND. THE B06 DEPARTED TOWARDS THE DEPARTURE END OF RUNWAY 12R, TURNED SOUTHEAST CROSSING OVER TAXIWAY NOVEMBER THEN TURNED NORTH PASSING QUEBEC AND STARTED TO AND CROSS RUNWAY 12R WHEN PILOT OBSERVED THE B737 AND TURNED BACK TOWARDS THE SOUTHWEST. ASDE-X ANALYSIS INDICATED THE CLOSEST PROXIMITY OF THE TWO AIRCRAFT WAS WHEN THE B06 WAS APPROXIMATELY 65 FEET FROM RUNWAY EDGE AT AN ESTIMATED 275 FEET VERTICAL AND 115 FEET FROM THE B737 WHO WAS AT AN ESTIMATED 100 FEET VERTICAL. THE B737 SAW THE B06 AND LEVELED OFF TO STAY BELOW THE HELICOPTER. A NMAC WAS FILED BY B737 PILOT ON 4/28/10. CLOSEST PROXIMITY REPORTED 100 FEET VERTICALLY AND 125 FEET LATERALLY.
