    AIRCRAFT 1 CROSSED RUNWAY 4L WITHOUT ATC AUTHORIZATION. GC INSTRUCTED AIRCRAFT 1/B772, SOUTHEAST BOUND ON TAXIWAY B, TO CROSS RUNWAY 31L AT TAXIWAY K. PILOT READBACK WAS CORRECT. LC CLEARED AIRCRAFT 2/B739 FOR TAKEOFF ON RUNWAY 4L. AIRCRAFT 1 DID NOT TURN RIGHT ONTO TAXIWAY K BUT TURNED LEFT ONTO TAXIWAY J AND CROSSED RUNWAY 4L WITHOUT APPROVAL. ASDE-X ALERTED; LC CANCELED AIRCRAFT 2 TAKEOFF CLEARANCE. THE AIRCRAFT CAME TO A STOP BEFORE TAXIWAY K4. THE CLOSEST PROXIMITY WAS APPROXIMATELY 1000 FEET.

    A BOEING B767 WAS INSTRUCTED TO TAXI TO RUNWAY 22R VIA TAXIWAY HOTEL AND LEFT ONTO TAXIWAY BRAVO. THE B767 DID NOT TURN LEFT ONTO BRAVO AS INSTRUCTED AND CROSSED RUNWAY 22R VIA HOTEL AND CONFLICTED WITH A DOUGLAS DC8 DEPARTING RUNWAY 22R ABEAM TAXIWAY CHARLIE. THE DC8 ROTATED AND FLEW OVER THE B767 BY LESS THAN 100 FEET. THE DISTANCE FROM CHARLIE TO HOTEL IS APPROXIMATELY 5,000 FEET. ASDE COMPLETELY WASHED OUT FROM HEAVY RAINS, SHOWED NO TARGETS. AMASS DID NOT ALERT. FIRST OFFICER ON DC8 REPORTED SEEING THE B767 HOWEVER THEY WERE PAST V1 (DECISION SPEED) AND CONTINUED ROTATION AT VR (ROTATION SPEED). THEY INCREASED THEIR ROTATION ANGLE TO GET BETTER CLIMB. FIRST OFFICER REPORTED OVERFLIGHT AT ABOUT 75 FEET.
