B777 LANDED RUNWAY 24R AND WAS ROLLING OUT WHEN LOCAL CONTROL 2 (LC 2) ISSUED B737 TAKEOFF CLEARANCE ON RUNWAY 24L. B777 WAS INSTRUCTED TO TURN LEFT AT TAXIWAY ALPHA-ALPHA AND HOLD SHORT OF RUNWAY 24L. B777 PILOT READ BACK CROSS RUNWAY 24L AND LC 2 DID NOT ATTEMPT TO CORRECT THE INCORRECT READ BACK. AS THE B737 WAS DEPARTING RUNWAY 24L THE B777 TAXIED ACROSS THE HOLD BARS ON TAXIWAY AA AND EVENTUALLY CONTINUED ACROSS RUNWAY 24L. B737 ROTATED JUST WEST OF TAXIWAY YANKEE AT AN APPROXIMATE SEPARATION DISTANCE OF 3075 FEET FROM THE B777 CROSSING AT TAXIWAY ALPHA-ALPHA. B737 PILOT REPORTED HE HAD TO CLIMB AT A HIGHER THAN NORMAL RATE TO CLEAR THE B777. A NON-FALSE ALERT WAS GENERATED BY THE AMASS AFTER CONFLICT DETECTION. B737 OVERFLEW THE B777 WITH A MODE-C READOUT OF APPROXIMATELY 494 FEET AND AN ESTIMATED VERTICAL COMPONENT OF 385 FEET. THIS EVENT OCCURRED AT HOT SPOT 1. RWSL WAS DECOMMISSIONED; RELS AT TAXIWAY ALPHA-ALPHA/RUNWAY 24L INTERSECTION COULD NOT ILLUMINATE.

A BOEING B737 LANDED ON RUNWAY 24R, EXITED AT TAXIWAY YANKEE THEN PILOT SWITCHED FREQUENCY TO GROUND CONTROL (GC) WITHOUT AUTHORIZATION. THE B737 CALLED GC ON YANKEE BETWEEN RUNWAY 24R AND RUNWAY 24L AND GC ISSUED TAXI INSTRUCTIONS ECHO TO THE GATE THINKING LOCAL CONTROL (LC) HAD TOLD THE B737 TO CROSS RUNWAY 24L. AN AIRBUS A320 WAS CLEARED FOR TAKEOFF ON RUNWAY 24L FULL LENGTH. THE B737 PILOT OBSERVED THE LIGHTS OF THE A320 AND QUESTIONED IF THEY WERE CLEARED TO CROSS. GC TOLD THEM NO AND TO "HOLD THERE, HOLD THERE". THE B737 USED NORMAL BREAKING BUT HAD STOPPED PASSED THE HOLD LINE BUT WAS SHORT OF THE RUNWAY EDGE. LC ELECTED TO ALLOW THE A320 TO CONTINUE TAKEOFF DUE TO THE SPEED OF THE AIRCRAFT. CLOSEST PROXIMITY ESTIMATED AT 50 FEET AS THE A320 PAST YANKEE. THE B737 STOPPED APPROXIMATELY 93 FEET FROM RUNWAY CENTERLINE, 18 FEET FROM RUNWAY EDGE. WINGSPAN OF A320 IS 112 FEET, RUNWAY WIDTH OF 150 FEET WHICH WOULD PUT CLOSEST HORIZONTAL PROXIMITY OF WINGTIP TO AIRCRAFT NOSE AT AN ESTIMATED 37 FEET.

AN EMBRAER E120 LANDED RUNWAY 24R AND WAS INSTRUCTED "NO DELAY TO HIGH-SPEED ZULU" WHICH THE PILOT ACKNOWLEDGED. TRAFFIC WAS AN AIRBUS A346 ON SHORT FINAL RUNWAY 24R. LOCAL CONTROL 2 (LC2) TOLD THE E120 PILOT TO "KEEP IT ROLLING, TURN LEFT OFF THE RUNWAY". THE E120 PILOT ACKNOWLEDGED AGAIN. LC2 OBSERVED THE E120 CROSSING THE RUNWAY EDGE LINE AS THE A346 WAS VERY SHORT FINAL AND INSTRUCTED THE E120 TO HOLD SHORT RUNWAY 24L AND AGAIN STATED "I SAID ZULU SIR, TURN LEFT OFF THE RUNWAY". THE E120 HAD TURNED LEFT TOWARDS TAXIWAY YANKEE INSTEAD OF CONTINUING ON ZULU. LC OBSERVED THE E120 THEN TURN RIGHT AND WAS UNSURE OF WHAT THE AIRCRAFT WAS GOING TO DO SO HE STATED "STOP RIGHT THERE". THE A346 WAS IN THE FLARE AND AMASS ALERTED. THE E120 WAS CLEAR OF RUNWAY 24R EDGE LINE BUT LC HAD STOPPED THE E120S FORWARD MOTION PRIOR TO CROSSING THE HOLD BAR. CLOSEST HORIZONTAL SEPARATION REPORTED WAS 50 FEET AS THE A346 PASSED TAXIWAY YZ. A346 WING SPAN IS 209 FEET, RUNWAY WIDTH IS 150 FEET. IF A346 WAS ON CENTERLINE, 29 FEET OF WINGTIP WOULD BE OVERHANGING PAST THE RUNWAY EDGE LINE.

A GULFSTREAM GLF5 WAS INSTRUCTED TO CROSS RUNWAY 25L AT GOLF AND HOLD SHORT OF RUNWAY 25R. THE GLF5 READ BACK HOLD SHORT THEN PROCEEDED TO CROSS 25R WITHOUT CLEARANCE AND CONFLICTED WITH A CANADAIR CRJ7 ON DEPARTURE 25R FROM TAXIWAT FOXTROT. THE CRJ7 ABORTED AND STOPPED APPROXIMATELY 100 FEET FROM GOLF AS THE GLF5 WAS HALFWAY ACROSS THE RUNWAY. DISTANCE FROM FOXTROT TO GOLF IS 3250 FEET. PILOT OF CRJ7 REPORTED BRAKES NEEDED A 15 MINUTE COOL DOWN. THIS EVENT MET THE PARAMETERS ESTABLISHED WITHIN AMASS TO ALARM SO AN ALERT WAS GIVEN. INCIDENT OCCURRED AT 1802L ON 9/30/06 BUT REPORT FORM USED ZULU REPORT DATE OF 10/1/06.

A CANADAIR CRJ2 LANDED RUNWAY 25L, EXITED AT TAXIWAY MIKE AND WAS INSTRUCTED TO HOLD SHORT RUNWAY 25R. READ BACK WAS CORRECT BUT THE CRJ2 ENTERED RUNWAY 25R EDGE AT MIKE WITHOUT AUTHORIZATION AND CONFLICTED WITH A EMBRAER E120 ON TAKEOFF ROLL SAME RUNWAY FROM ABEAM TAXIWAY GOLF (GOLF TO MIKE IS 3,200 FEET). THE CRJ2 STOPPED BEFORE FULLY ENTERING THE RUNWAY BUT WITH THE NOSE OVER THE EDGE LINE. LOCAL CONTROL SAW THE CRJ2S POSITION AND RELAYED THIS INFORMATION TO THE DEPARTING E120. THE E120 RECEIVED THE TRAFFIC, ESTABLISHED A VISUAL ON THE CRJ2, ROTATED EARLY (PAST KILO), AND WAS AIRBORNE PRIOR TO MIKE (KILO TO MIKE IS APPROXIMATELY 600 FEET). CLOSEST PROXIMITY WAS REPORTED AS 50 FEET HORIZONTAL.

AFTER POSITION RELIEF, LC, THINKING AAR 204 WAS LANDING RUNWAY 24R, INSTRUCTED SOUTHWEST (SWA) 440, B737, TO TIPH RUNWAY 24L AND BEGAN COORDINATING A DEPARTURE RELEASE. LC CLEARED SWA 440 FOR DEPARTURE. AAR 204, NOW ½ MILE FINAL, TOLD TOWER THEY WERE GOING AROUND DUE TO AN AIRCRAFT ON RUNWAY 24L. AMASS ALARMED AND LC GAVE GO AROUND INSTRUCTIONS TO AAR 204 AND INSTRUCTED SWA 440 TO ABORT DEPARTURE. TEN SECONDS AFTER AMASS ALARMED; AAR 204 OVER FLEW SWA 440 BY LESS THAN 200 FEET VERTICAL. SWA 440 HAD BEGAN TAKE-OFF ROLL, ABORTED AND EXITED TAXIWAY E-8.
