AIRCRAFT 2/B738 WAS ON FINAL RUNWAY 31C CLEARED TO LAND. AIRCRAFT 1 WAS TAXIING TOWARD RUNWAY 31C AS AIRCRAFT 2 REACHED THE RUNWAY 31C THRESHOLD. AIRCRAFT 1 CROSSED RUNWAY 31C AT RUNWAY 4L WITHOUT ATC AUTHORIZATION. AIRCRAFT 2 ANNOUNCED PILOT INITIATED GO AROUND PRIOR TO TAXIWAY Y. THE TECHNICIANS WERE OUTSIDE THE VEHICLE PERFORMING WORK ON THE PAPIS. THE PRESENCE OF THE VEHICLE AND PERSONNEL IN THE RUNWAY 31C RSA HAD BEEN COVERED IN THE GC PRB PRECEDING THE EVENT. GC ISSUED TAXI INSTRUCTIONS TO AIRCRAFT 1/B38M FOR RUNWAY 31C, TAXI VIA TAXIWAY T AND TAXIWAY E3. DURING THE PREVIOUS LC PRB, THE VEHICLES WERE MENTIONED BUT NOT THEIR PRESENCE IN THE RUNWAY 31C RSA. LC ISSUED LUAW AND THEN TAKEOFF CLEARANCE RUNWAY 31C TO AIRCRAFT 1 AND THE AIRCRAFT DEPARTED. ESTIMATED CLOSEST PROXIMITY BETWEEN AIRCRAFT 1 AND THE VEHICLE PARKED ON THE SHOULDER 30-40 FEET BASED ON B38M WINGSPAN.

A POSITION RELIEF BRIEFING WAS CONDUCTED TO COMBINE GC WITH CD WHERE THE GROUND CONTROLLER REFERENCED APPROVED COORDINATION TO CROSS THE RUNWAY 13R APPROACH HOLD. AIRCRAFT 1 OVERFLEW AIRCRAFT 2 INSIDE THE RPZ. CLOSEST PROXIMITY WAS 35 FEET VERTICAL AND 56 FEET LATERAL.

AIRCRAFT 1 COMMENCED A RIGHT TURN ACROSS RUNWAY 4R AFTER BECOMING AIRBORNE, AIRCRAFT 1 OVERFLEW RUNWAY 4R AT OR BELOW 200 FEET. THE EVENT OCCURRED WHEN AIRCRAFT 2 WAS ROLLING OUT, CROSSING RUNWAY 13R AND AIRCRAFT 1 WAS AIRBORNE AND OVER RUNWAY 13C, TURNING SOUTHEAST, ACROSS RUNWAY 4R. CLOSEST PROXIMITY IS UNKNOWN BECAUSE AIRCRAFT 1 DROPPED OFF THE ASDE WHILE PASSING OVER RUNWAY 4R.

THE PA46 WAS THEN CHANGED TO AND CLEARED TO LAND RUNWAY 22R IN ORDER TO GET A B737 DEPARTURE OFF. LOCAL CONTROL FORGOT ABOUT THE DV40 TIPH ON RUNWAY 22R (THE DV40 WAS TIPH FOR LESS THAN 2 MINUTES). THE PA46 LANDED OVER THE DV40. CLOSEST PROXIMITY ESTIMATED AT 50 FEET VERTICAL. TOUCHDOWN POINT IS UNKNOWN. THERE IS A DISPLACED THRESHOLD 880 FEET DOWN RUNWAY 22R. TAXIWAY PAPA IS APPROXIMATELY 192 FEET BACK FROM THE DISPLACED THRESHOLD. ATCT CONTROLLERS IN THE TOWER AT THE TIME DID NOT SEE THE FLY OVER. TOWER HAS CLEAR LINE OF SIGHT TO APPROACH END RUNWAY 22R/L.

THE H25C WAS SOUTHEAST ON TAXIWAY FOXTROT WHEN ATCT INSTRUCTED THE H25C TO CROSS RUNWAY 4R. ATCT REALIZED A MCDONNELL-DOUGLAS MD80 HAD BEEN CLEARED FOR DEPARTURE ON RUNWAY 22L SO HE ISSUED HOLD SHORT INSTRUCTIONS TO ANOTHER AIRCRAFT FOR RUNWAY 4R/22L INTENDING TO ISSUE THE HOLD SHORT INSTRUCTIONS TO THE H25C. THE H25C CROSSED RUNWAY 22L VIA FOXTROT AND IN FRONT OF THE MD80 WHO WAS ROTATING AT THE INTERSECTION OF RUNWAY 31R AND 22L. CLOSEST HORIZONTAL SEPARATION REPORTED AS 1,078 FEET. THE H25C PILOT DISPUTED THE 1,078 FEET BECAUSE HE STATED "AS SOON AS HIS TAIL CLEARED THE RUNWAY, HE HEARD THE MD80 PASS OVERHEAD".
