AS THE AIRCRAFT APPROACHED THE HOLD LINE, THE PILOT ADVISED THE CONTROLLER THEY WERE HOLDING SHORT OF TAXIWAY TANGO, TO WHICH THE CONTROLLER RESPONDED HOLD POSITION FOR LANDING TRAFFIC. E145 CONTINUED AND TURNED SOUTH ON TAXIWAY TANGO. A SECOND E145 WAS NOT SENT AROUND AND OVERFLEW THE TAXIING AIRCRAFT WHILE LANDING RUNWAY 9R. THIS OCCURRED AT HOT SPOT 6. CLOSEST PROXIMITY ACCORDING TO ASDE ANALYSIS WAS 14 FEET LATERAL AND 76 FEET VERTICAL.

    AIRPORT VEHICLE ENTERED RUNWAY 32R AT THE APPROACH END OF THE RUNWAY WITHOUT AUTHORIZATION AND GENERATED AN ASDE ALERT WITH B744 ON SHORT FINAL. ATC SENT THE B744 AROUND AND ATTEMPTED TO CONTACT THE VEHICLE WITHOUT SUCCESS. ASDE-X ANALYSIS INDICATED THAT THE B744 BANKED SLIGHTLY TO THE LEFT AND THE VEHICLE DRIVER WAS PULLING OFF THE RUNWAY BETWEEN TAXIWAYS HOTEL 2 AND HOTEL. CLOSEST PROXIMITY WAS DETERMINED TO BE 95 FEET VERTICAL AND LESS THAN 100 FEET LATERAL.

    GROUND CONTROL INSTRUCTED CRJ7 TO TAXI TO RUNWAY 28R AT TAXIWAY ECHO ECHO VIA TAXIWAYS HOTEL, UNIFORM, ALPHA, FOXTROT, AND NOVEMBER, AND MONITOR THE TOWER FREQUENCY. CRJ7 PILOT REPORTED HE HAD BECOME DISTRACTED AND MISSED THE TURN ONTO TAXIWAY NOVEMBER, DID NOT SEE THE WIG WAG LIGHTS AND ENTERED RUNWAY 28R AT TAXIWAY FOXTROT. E145 BEGAN ITS DEPARTURE ROLL ON RUNWAY 28R FROM INTERSECTION TAXIWAY ECHO ECHO AS THE CRJ7 WAS CROSSING THE HOLD SHORT MARKING FOR RUNWAY 28R. ASDE-X ALERTED AS THE CRJ7 ENTERED THE RUNWAY WITH THE E145 APPROACHING THE INTERSECTION WITH TAXIWAY FOXTROT. E145 NOSE WHEEL WAS OFF THE GROUND AND THE MAINS WERE ABOUT TO LIFT. CRJ7 TURNED LEFT ONTO RUNWAY 28R. E145 PILOT REPORTED THAT HE HAD ENOUGH RUDDER CONTROL TO MANEUVER TO THE LEFT SIDE OF THE RUNWAY. ESTIMATED CLOSEST PROXIMITY WAS UNDETERMINED. BASED ON AIRCRAFT AND RUNWAY DIMENSIONS, SEPARATION WAS APPROXIMATED AT 10 FEET OR LESS.

NORTH GROUND CONTROL (NGC) TAXIED THE A320 SOUTHBOUND ON TAXIWAY TANGO WITH INSTRUCTIONS TO GIVE A GOOD RATE AROUND THE CORNER, TRAFFIC TWO AND A HALF MILES OUT ON FINAL. SOUTH LOCAL CONTROL WARNED NLC OF THE POSSIBLE OVERFLY SITUATION BUT B738 WAS ALREADY PAST THE TAXIWAY AND LANDED RUNWAY 9R. ACCORDING TO THE ASDE-X ANALYSIS, THE B738 WIN PASSED APPROXIMATELY 10 FEET BEHIND THE TAIL OF THE A320 AND 95 FEET ABOVE THE TAXIWAY.

    FIRST CRJ7 CALLED GROUND CONTROL (GC) AT TAXIWAY GOLF, HOLDING SHORT OF TAXIWAY TANGO. GC ISSUED INSTRUCTIONS TO CRJ7 TO TAXI VIA TAXIWAYS TANGO, ALPHA 7 AND BRAVO. CRJ7 WAS ON TAXIWAY TANGO AND CROSSING THE EXTENDED CENTERLINE FOR RUNWAY 9R PROTECTED AREA APPROXIMATELY 1327 FEET FROM THE RUNWAY THRESHOLD WHILE A SECOND CRJ7 WAS LANDING . THIS AREA IS PROTECTED FOR ALL AIRCRAFT IN ALL WEATHER CONDITIONS DURING EASTBOUND OPERATIONS. TAXIING CRJ7 PASSED THE EXTENDED RUNWAY CENTERLINE JUST PRIOR TO THE ARRIVING CRJ7 REACHING THE TAXIWAY AND ASDE-X ANALYSIS INDICATES THE WING OF THE ARRIVING CRJ7 PASSED APPROXIMATELY 85 FEET ABOVE THE TAIL OF THE TAXIING CRJ7.

RUNWAY 22L THRESHOLD TO RUNWAY 28 THRESHOLD HORIZONTAL DISTANCE IS APPROXIMATELY 800 FEET.

    TRAINING IN PROGRESS ON NORTH LOCAL (NLC) POSITION. A EMBRAER E145 WAS CLEARED FOR TAKEOFF ON RUNWAY 32L ABEAM TAXIWAY T10 BY LOCAL 3 (LC3) WITH A EMBRAER E135 ON AN APPROXIMATELY 3.5 MILE FINAL FOR RUNWAY 9R. THE FLIGHT PATHS FOR RUNWAY 32L AND 9R INTERSECT. LC3 DID NOT SEE OR WAS AWARE OF THE RUNWAY 9R ARRIVAL. UPON RECOGNIZING THE CONFLICT, LC3 ISSUED THE DEPARTING E145 THE CROSSING E135 TRAFFIC AND INSTRUCTED THE PILOT TO STAY AS LOW AS YOU CAN. E145 PILOT STATED "YEAH, WERE DOING THAT". LC3 THEN INSTRUCTED NLC TO SEND THE E135 AROUND. THE E135 CROSSED OVER RUNWAY 32L BY TAXIWAY T2 AND STARTED A CLIMB WHEN THE E145 WAS JUST SOUTH OF T2 AND BECOMING AIRBORNE. CLOSEST PROXIMITY ESTIMATED AT THAT POINT WAS 125 FEET VERTICAL, 350 FEET LATERAL. THIS EVENT DID NOT MEET THE PARAMETERS ESTABLISHED WITHIN ASDE-X (NON-INTERSECTING RUNWAYS) TO CAUSE AN ALARM AND NO ALERT WAS RECEIVED. E145 PILOT STATEMENT: DEPARTING 32L AT T10 AT CHICAGO O'HARE WE BEGAN THE TAKEOFF ROLL NORMALLY. I CALLED 80 KNOTS AND CONTINUED TO SCAN INSIDE AND OUT. AT ABOUT 115 KNOTS I LOOK DOWN TO CHECK FOR V1 SPEED AND NOTICE AN AMBER TARGET 200 ABOVE US ON THE TCAS. I MOVED FORWARD TO LOOK AROUND THE WINDOW POST AND NOTICE AN EAGLE E145 ON SHORT FINAL FOR 9R. I QUICKLY PUT MY HAND ON THE YOKE TO DELAY THE ROTATION AND POINTED OUT THE TARGET TO MY FO. HE ASKED IF WE SHOULD ABORT BUT WE WERE PAST V1 AND RUNNING OUT OF PAVEMENT, I SAID NO JUST KEEP ROLLING AND ROTATE PAST HIS ARRIVAL PATH. ATCT THEN GAVE US A TRAFFIC ALERT AND TOLD US TO STAY LOW, THAT ADVICE MAY HAVE BEEN TO LATE IF WE DIDN'T SEE HIM FIRST. HE MAY HAVE CLEARED THE AIRCRAFT BY 150 FT AT THE POINT HE CROSSED OVER. WE CONTINUED OUR CLIMB AND THE FLIGHT WITHOUT INCIDENT.

AN AIRBUS A320 WAS INSTRUCTED TO LINE UP AND WAIT (LUAW) ON RUNWAY 22L FULL LENGTH AND EXPECT AN IMMEDIATE TAKEOFF CLEARANCE FOR ANOTHER DEPARTURE, THE MD82 ON THE SAME RUNWAY THAT WAS NOT YET 6000’ AND AIRBORNE. LOCAL (LC) WAS ATTEMPTING TO GET THE A320 TO DEPART PRIOR TO AN ARRIVAL, A BOEING B744 FOR RUNWAY 28. THE A320 STARTED DEPARTURE ROLL AS THE B744 FLEW OVER THE AIRCRAFT. CLOSEST VERTICAL SEPARATION REPORTED WAS 80 FEET VERTICAL, 0 LATERAL. THE B744 PILOT REPORTED TO ATCT OF FLYING OVER THE DEPARTING AIRCRAFT.
