Incident Details
During the landing roll, at a speed of approximately 30 knots, the pilot applied the brakes in preparation for turning off the runway. Immediately, the right brake pedal went to the floor. The pilot attempted to pump the pedal to restore pressure, but these efforts were unsuccessful. The aircraft subsequently ran off the left side of the runway.
Ditch Impact
Once off the runway, the main landing gear impacted two erosion ditches. These ditches were approximately 16 inches deep and 25 feet long. Notably, the ditches were not marked, and there were no Notices to Air Missions (NOTAMs) issued regarding their presence.
Brake System Failure
A post-accident examination of the brake system revealed that the right brake cylinder had failed. This failure prevented the right brake from engaging, leading to asymmetric braking and the subsequent loss of directional control.
