A passenger on board recounted that when takeoff power was initially applied, the windshield became fogged to such an extent that it completely obscured forward visibility. The passenger specifically noted this occurred at the start of the takeoff.

On-Site Examination
An on-site investigation conducted after the incident revealed that the aircraft had traveled in a straight line from the point where takeoff began. Furthermore, no tire marks were found on the runway surface, which would have been indicative of braking application. The investigation did not identify any evidence suggesting that braking was used during the event.
