Accident Summary
The accident occurred shortly after takeoff from a runway covered with slush. The aircraft had climbed to approximately 200 ft above ground level when it began rolling to the left. The pilot attempted to correct the roll with right aileron and rudder inputs, but the aircraft did not respond. The aircraft subsequently crashed.
Pilot Report
The pilot stated that the preflight inspection, engine start, taxi, and engine run-up were all normal. The takeoff from the slush-covered runway was described as routine. However, after reaching an altitude of about 200 ft, the aircraft began rolling left. Despite the pilot's corrective inputs, the roll continued without response.
Post-Crash Examination
A thorough examination of the wreckage, flight controls, and control cables was conducted. The examination found no evidence of any preimpact failure or malfunction. The control system appeared intact and functional at the time of impact.
