Incident
A pilot initiated a takeoff without obtaining a clearance from the tower. The tower controller observed the aircraft depart, immediately turn left, and collide with a 10-foot dirt bank located between the parallel runways. After striking the dirt bank, the aircraft skidded across an adjoining taxiway before coming to rest.
Injuries and Pilot Statements
The pilot sustained serious injuries and was hospitalized. Prior to his death on January 2, 1984, he told his wife that the engine had quit during takeoff and that he switched the fuel selector valve in an attempt to restart the engine. He reportedly stated that he tried to set the aircraft down on a parallel runway. Additionally, both his wife and a police officer reported that the pilot told them he had "blacked out" during the takeoff sequence.
Investigation Findings
Examination of the aircraft revealed the main fuel selector was found in the OFF position. No engine malfunction was discovered. An autopsy found evidence of deep vein thrombosis with a pulmonary embolus, along with generalized cerebral arteriosclerosis, coronary arteriosclerosis, and arterial nephrosclerosis.
