Event
During initial climb, at an altitude of approximately 100 feet above ground level, the engine ceased operation. A witness reported that the aircraft then turned to the right, subsequently turned to the left, and nosed down into the ground.
Post-Accident Inspection
An examination of the wreckage revealed ice crystals on the left wing at the location where the fuel tank had ruptured and fuel was leaking. The witness also stated that ice crystals were present on the ground where fuel was leaking. A few drops of water were found in the engine quick drain. However, fuel samples taken from the wing tank fuel drains contained no water.
Fuel System Condition
The area around both fuel caps was rusty, and the cap seals showed rust marks. The pilot stated that the left fuel tank could not be filled because it leaked. No mechanical failure or mechanical issue could be identified during the investigation.
