Accident Description
During the initial takeoff climb, the aircraft experienced a loss of control and collided with Runway 23, approximately 120 feet from the departure end. The aircraft was observed to enter a steep climb shortly after takeoff, followed by the right wing dropping. The aircraft then entered a spin before contacting the runway.
Witness Observations
Several individuals reported seeing the pilot perform an abrupt pull-up during takeoff on various occasions preceding the accident flight. These observations align with the steep climb noted on the accident flight.
Maintenance Findings
Examination of the aircraft and its maintenance records revealed that Airworthiness Directive (AD) 83-14-08, which concerns the installation of additional flow strips on the wings, had not been complied with. The non-compliance was identified during the post-accident inspection.
