Incident Overview
A helicopter flight originating from Madras, Oregon, or destined for Walla Walla, Washington, experienced a loss of engine power due to a fuel flow problem. The pilot executed a successful autorotation near Warm Springs, Oregon. Following the event, a company mechanic was dispatched to the helicopter and repaired a loose fuel line fitting.
Subsequent Events
Late that evening, the pilot started the engine and ran it for approximately 10 minutes. The helicopter was then hovered for 4 to 5 minutes before commencing departure. During the departure, at about 40 feet above ground level (AGL), the engine began losing revolutions per minute (RPM). The pilot attempted to restore power but was unable to do so. The helicopter subsequently impacted the ground on its skid landing gear and tail skid, then came to rest on its left side approximately 60 feet from the initial impact point.
Examination Findings
An examination of the helicopter's fuel system revealed that the fuel strainer (model KOE/K2250B-2) was loose at both ends of the glass and could be turned with slight pressure. Additionally, the bolt at the bottom of the lower piece was not safety wired and was loose.
