Accident Overview
A Bell 206B helicopter, operated by a company mechanic's employer, was conducting forestry operations with a fire dispensing apparatus slung beneath the aircraft. While returning to the helipad for fuel, the pilot encountered a problem with the apparatus. He landed, released the external load, and exited the aircraft to disconnect an electrical connection. The accident occurred when the pilot resumed flight without noticing that a cable from the released load was caught across his right skid.
Sequence of Events
According to the pilot, who holds an airline transport rating, he initiated a climbing right turn after takeoff. As the external load was inadvertently lifted, it immediately began a severe oscillation, causing a loss of control. The aircraft impacted small trees before striking the ground. A company mechanic who witnessed the accident stated that he attempted to alert the pilot before takeoff but was unable to get his attention.
Injuries and Damage
The helicopter sustained substantial damage as a result of the impact. The pilot was seriously injured. Visual meteorological conditions prevailed at the time of the accident.
