Accident Sequence
The aircraft lifted off from the top of a support truck and commenced a takeoff with a partial load of chemicals intended for application to a logged over forest area. Approximately 10 to 15 feet after lift-off, the pilot detected a loss of rotor RPM. The aircraft was at the edge of a ravine with steep sides, and no suitable landing area was available. The pilot descended along the side of the ravine and executed a modified autorotation into the tops of trees at the bottom of the ravine. The ground support man reported seeing a puff of smoke around the time the pilot noted the power loss.
Examination and Testing
The aircraft was recovered and examined. No malfunctions were discovered that would have prevented the engine from running. The engine was removed and shipped to the factory, where it was run in a test cell. The engine performed well during testing.
Prior Installation History
A records check revealed that the engine had been shipped to Bell Helicopter as a new '0' time engine. It was installed in the Hughes approximately 19 months prior to the accident. The installation was inspected and approved by the FAA.
