What happened
Prior to commencing the takeoff roll, the pilot noticed an asymmetrical deflection of the rudder and brake pedals. Specifically, the left brake pedal was positioned inward approximately 1 1/2 inches farther than the right brake pedal. Despite this observation, the pilot proceeded with the flight.
Upon landing, the pilot attempted to execute a right turn to depart the runway. However, he was unable to stop the aircraft from continuing its rightward rotation. The airplane ground-looped, resulting in damage to the left wing and the landing gear. The pilot later stated that he had attempted to use the left brake to counteract the ground loop but was unsuccessful.
The investigation
Following the accident, a maintenance overhaul was performed on the aircraft. During this process, it was determined that the left brake, part number 0341005-65, was worn beyond allowable tolerances. This mechanical failure directly contributed to the inability of the pilot to maintain directional control during the post-landing maneuver.
Findings
The primary factor in this incident was the excessive wear of the left brake assembly. The pilot's pre-flight observation of the pedal asymmetry indicated a known issue that was not addressed prior to flight. The failure of the braking system on one side prevented effective correction of the ground loop, leading to substantial airframe damage.
