Incident Overview
A helicopter flight operating out of Kailua-Kona Airport (KOA) experienced an engine failure while flying about one-quarter mile offshore at 200 feet above ground level. The pilot executed an autorotation into the ocean, and the helicopter sank. All occupants were able to exit the aircraft, but one passenger drowned. The helicopter was not equipped with the required flotation devices and was not recovered.
Sequence of Events
According to the investigation source, the pilot first picked up two passengers at a resort hotel before flying to Kailua-Kona Airport. After a hot refueling with those two passengers still on board, two additional passengers boarded. The flight then departed KOA. While exiting Waimanu Valley over the beach, the pilot observed another company helicopter. After establishing radio communication with that helicopter, the pilot altered course westward, extending about one-quarter mile from the shoreline at 200 feet above ground level. Shortly thereafter, the helicopter's engine began to decelerate, prompting the pilot to perform an autorotation landing on the water.
Occupant Outcome
The helicopter sank, but all individuals on board exited the aircraft. However, the passengers were not wearing life vests, and one passenger drowned as a result.
Equipment Deficiencies
The investigation source indicates that the helicopter was not equipped with any of the required flotation devices.
Aircraft Status
The helicopter was not recovered after the accident.
