Incident Overview
A helicopter rolled over as it was lifting off from a transport trailer. The incident occurred during the takeoff attempt. Following the rollover, a post-crash examination was conducted.
Examination Findings
The post-crash examination revealed that the left rear tie-down strap on the transport trailer was wrapped around one of the helicopter's skids. Evidence indicated that the strap, which was attached to the trailer, had not been unhooked properly. The strap remained attached to the trailer during the lift-off, becoming entangled with the skid and causing the helicopter to roll over.
Crew Actions and Statements
The ground crewman reported that he believed he had removed all tie-down straps prior to the attempted takeoff. The pilot stated that he had visually checked all straps during his preflight inspection. However, the left rear strap was not properly unhooked. The pilot later acknowledged that a hands-on inspection of the strap may have prevented the occurrence.
