Accident Details
A helicopter on an instructional flight with a student and a certified flight instructor (CFI) was involved in an accident while practicing emergency maneuvers. The crew had decided to perform an autorotation and simulate a tail rotor failure.
Sequence of Events
According to reports, the helicopter made a smooth touchdown during the maneuver. However, shortly after landing, the aircraft began to vibrate. In response, the CFI elected to get the helicopter airborne again. The instructor later stated that there was insufficient rotor RPM to maintain flight. Subsequently, the helicopter made two 360-degree turns to the right and contacted the ground.
Investigation Findings
No preimpact failure or malfunction of the helicopter was reported. The investigation did not identify any mechanical issues prior to the accident.
