Factual Narrative
After the pilot had landed, the aircraft was serviced with 58.9 gallons of fuel, leaving the tanks nearly full to a capacity of 60 gallons. On the subsequent takeoff, the aircraft remained on the runway for approximately 2,500 feet of the available 3,000 feet. After lift-off, the aircraft entered a right turn and reached a reported altitude of only about 100 feet.
A witness reported that the engine was not running at full power and that dark gray smoke was coming from its exhaust. The witness also stated that the aircraft stalled as the pilot was apparently attempting to turn back. The aircraft subsequently crashed onto the roof of an unoccupied church school.
Investigation Findings
An examination of the aircraft revealed that both fuel tanks were contaminated with water and sand. Water and sand were also found in the fuel filter and carburetor finger strainer. Both fuel caps were tested and found to leak when water was applied to the upper wing surface.
The manufacturer (Beech) had issued a Class I service bulletin for the installation of improved fuel tank caps; however, the improved fuel caps had not been installed on the accident aircraft. Additionally, corrosion was evident in the wing sump and fuel filter drains, making the latter difficult to operate.
