Overview
Following an annual inspection, a pilot prepared an aircraft for a test flight. Three passengers were loaded aboard, and the pilot commenced run-up checks. During the run-up, the left engine ran rough. After the roughness cleared, the pilot initiated the takeoff. However, as the aircraft accelerated down the runway, the left engine ran rough again, prompting the pilot to abort the takeoff.
Second Takeoff Attempt
The pilot performed another run-up check and attempted a second takeoff, which proceeded without incident. As the aircraft climbed through 300 feet above mean sea level (MSL), the right engine began to sputter. The pilot continued the climb to 500 feet MSL before reducing power on the right engine, shutting it down, and feathering the propeller.
Descent and Impact
With the right engine inoperative, the aircraft descended as the pilot initiated a turn toward the airport. The aircraft subsequently impacted marsh ground approximately 2.75 miles from the airport.
Post-Accident Examination
Examination of the aircraft after the accident revealed the presence of water and rust particles on the injector diaphragm and on the fuel inlet screen. An Airworthiness Directive (AD) had been issued for this aircraft model, requiring daily and post-refueling checks of the fuel system by draining to avoid the accumulation of water and sediment.
